Tommijean worked as a psychologist in private practice and was a professional dancer when he was young. Thomas, 82, has been involved in computer engineering for 40 years.

Now, Thomas wants 79-year-old Tomijan to be at home.

After that particularly terrible fall, he was told it might be time to start investigating end-of-life care. He contacted Casa de la Luz Hospice, but after all, she didn’t need the hospice. However, she needed palliative care to help manage her medical and cognitive tasks.

Soon after, through Casa de Lars, social workers named Kuhn and Laura Islemer began visiting Thomas’s home. They still provide that care to the couple, but now through Elder Health, where the couple joined in April. Last week, Doctor Self called her first home.

As a palliative care worker, Aylmer assists Thomas by providing support and helping to navigate all the information and options related to wife care. She says her training has taught people how to “save space” when they experience difficult times and how to keep their loved ones at home.

“At the heart of it is the attentiveness, compassion, and interest in the people and family in front of me,” Alymer said. After understanding their story and what is important to them, she helps to develop a “plan of care that will improve quality of life and instill well-being as much as possible along the path of a difficult journey.” ..
